Colorado health care professionals and patient advocates are working together to make Colorado the safest state for all patients. Many errors are caused by breakdowns in the system. Errors or near misses should prompt an examination of how to prevent them from occurring again. Currently, the patient and doctor relationship is hindered by a culture of blame that undermines the goal of achieving the safest and highest quality health care.
